Everything here is written from sources, and sources get misread. If you find an error, it gets fixed.
What counts as an error
- A wrong figure.
- A claim attributed to a study that does not actually support it.
- A source that has been retracted, corrected, or superseded.
- A mechanism described incorrectly.
- A trial status that has changed since the entry was written.
- A dead or redirected link.
Also, and this one matters: anything stated more confidently than its evidence justifies. That is an error on this site even though most sites would not treat it as one.
How to report one
Use the contact form. Say which entry, which sentence, and what the source actually says. A link to the source helps more than anything else you could include.
You do not need to be polite about it.
What happens next
The claim gets checked against the source it cites.
If it is wrong, it is corrected and the entry’s modified date changes. Where the correction is material, meaning a figure, an attribution, or a claim a reader could have relied on, a note stays on the entry saying what changed and when. Minor fixes such as a broken link or a typo are made without a note.
If the claim turns out to be right, you get a reply explaining why, with the source.
Three different dates
Entries show when they were published, when they were last modified, and when their sources were last checked against the originals. Those are three separate things and this site keeps them separate.
Dates are never bumped to make a page look fresh.
