Petrelintide Approval Timeline: When Could It Reach the Market?

October 31, 2026

Petrelintide is not approved anywhere, and the earliest dates in the trial registry point to 2029 at the soonest for any approval. This page lays out the milestones so far, the Phase 3 calendar, and the arithmetic behind that estimate, with the limits stated.

Ask PeptideBlog about any peptide

Quick answer: Petrelintide is an investigational drug. Zealand Pharma and Roche have not announced a marketing application, and the three Phase 3 ZUPREME trials are still recruiting, with registered primary completion dates from October 2028 to July 2030.[6][7][8] The FDA sets review goals of 6 months for priority review and 10 months for standard review.[10] Added together, those dates make any approval before 2029 hard to square with the registry. That is arithmetic, not a forecast.

Search interest in “petrelintide approval” has grown since the first Phase 3 trials opened. This page documents what is on the public record: sponsor announcements, a peer-reviewed paper, and the ClinicalTrials.gov registry. It does not predict an approval date. For the trial results themselves see the petrelintide amylin drug comparison, the dosage guide and the side-effects guide. For the drug profile, see the petrelintide research entry.

Where petrelintide stands

Petrelintide is a once-weekly amylin analogue developed by Zealand Pharma, with Roche as its Phase 3 partner. It has finished two Phase 2 trials, ZUPREME-1 in adults with obesity and ZUPREME-2 in adults with obesity and type 2 diabetes, and the Phase 3 program is recruiting.[4][5][6] No regulator has approved it. Zealand’s October 2026 announcement of the ZUPREME-2 topline said nothing about a regulatory submission.[5]

The Phase 3 calendar

The three Phase 3 trials share a primary endpoint of percent change in body weight at week 64, and all list Roche as sponsor with Zealand Pharma as collaborator. Together they plan about 7,000 participants.[6][7][8][5]

ZUPREME-5NCT07843472Overweight or obesity with established cardiovascular disease2,500July 2030

Registry dates are sponsor projections, are revised often, and are not commitments. Evidence category: trial registry (no results posted).

“Primary completion” means the date the last participant is examined for the main endpoint. Results come after that, because the data must be cleaned, locked and analyzed. ZUPREME-5 has the longest horizon, probably because a cardiovascular trial has to follow people long enough to count heart events.

How long FDA review takes

The FDA sets a review goal of 6 months for applications granted priority review and 10 months for standard review. It tells the sponsor which one applies within 60 days of receiving the application.[10] The FDA page does not say exactly when the clock starts for each type, so this page does not add or subtract months for filing steps.

A real example shows that goals and outcomes are different things. Novo Nordisk announced on 18 December 2025 that it had submitted a new drug application for CagriSema (cagrilintide plus semaglutide), and said the FDA was “expected to review the CagriSema application in 2026,” without giving a decision date.[11] When Drugs.com’s status page was last updated on 26 October 2026, it still listed CagriSema as not FDA approved.[12] That is a secondary source, so the FDA’s own records are the place to confirm any status. See the CagriSema entry for what that combination has shown in trials.

A floor, not a forecast

Here is the arithmetic. It uses only registry dates and the FDA’s published review goals, and every step in between is unknown to outsiders.

How the registry dates stack up
StepWhat the sources sayWhat is not known
Earliest Phase 3 primary completionOctober 2028 (ZUPREME-4)[7]Whether it holds. These dates move.
Main obesity trial finishesDecember 2028 (ZUPREME-3)[6]Whether a filing waits for it.
Data analysis and filing preparationNot publishedHow many months. The sponsors have not said.
FDA review6 months (priority) or 10 months (standard) as goals[10]Which applies, and whether the review is extended.
Cardiovascular trialJuly 2030 (ZUPREME-5)[8]Whether the sponsors will wait for it before filing.

If the sponsors filed on the strength of ZUPREME-3 and ZUPREME-4 alone, the earliest registered completion is October 2028. Add time to analyze and file, then 6 to 10 months of review, and the earliest approval lands in 2029. If they wait for ZUPREME-5, it moves to 2030 or later. Neither sponsor has said which path it will take. Treat 2029 as a floor, because it assumes every date holds and nothing goes wrong, and it is not a prediction.

This page covers FDA review goals only. Other regulators have their own processes and timelines, and this page does not estimate them.

How eloralintide compares

Eli Lilly’s eloralintide is the other amylin drug in Phase 3. Lilly has six registered ENLIGHTEN trials planning 8,615 participants, and the earliest registered primary completion among them is January 2028.[13] That is earlier than the earliest petrelintide date (October 2028). Registry dates only describe plans, and neither company has announced a filing. See the eloralintide entry for its Phase 2 results, and the amylin drug comparison for how the two programs differ.

The Roche combination trial

Roche has registered a Phase 2 dose-finding trial of petrelintide together with enicepatide (CT-388), its once-weekly GLP-1 and GIP receptor agonist. It plans 486 adults, was listed as not yet recruiting on 30 October 2026, and gives a primary completion of November 2027.[9] Zealand described it as planned for the second half of 2026.[5] No results exist, so a combined product is a hypothesis at this stage, not a timeline. The enicepatide entry covers that drug alone.

What would change the timeline

  • Faster or slower enrollment. The registry dates assume recruitment goes to plan.
  • A filing before all three trials finish. Sponsors sometimes file on part of a program. Nothing in the sources reviewed says whether Zealand and Roche will.
  • Priority review. The FDA decides this within 60 days of receipt, which would shorten the review goal from 10 months to 6.[10]
  • Safety findings. Any new finding in larger, longer trials could change the plan. Long-term safety is unknown today, as covered in the side-effects guide.

The petrelintide news and readouts tracker logs each announcement and registry change as it happens.

Frequently asked questions

Is petrelintide FDA approved?

No. Petrelintide is investigational and has not been approved by the FDA or any other regulator. Zealand Pharma and Roche are running Phase 3 trials and have not announced a marketing application.

When will petrelintide be approved?

Nobody can say. Registered Phase 3 primary completion dates run from October 2028 to July 2030, and the FDA sets review goals of 6 to 10 months, so any approval before 2029 is hard to square with the registry. That is arithmetic from projected dates, not a forecast.

When do the petrelintide Phase 3 trials finish?

The registry lists primary completion in October 2028 for ZUPREME-4, December 2028 for ZUPREME-3 and July 2030 for ZUPREME-5. Registry dates are projections and are often revised.

How long does FDA review take?

The FDA’s review goal is 6 months for priority review and 10 months for standard review. It communicates the designation within 60 days of receiving an application. Actual timing can differ, as the long wait on CagriSema shows.

Is petrelintide approved in Europe or elsewhere?

No approval has been announced anywhere. This page covers FDA goals only and does not estimate other regulators’ timelines.

Can you buy petrelintide?

Not as an approved medicine. Some research suppliers list it for laboratory use, including this coming-soon research listing. Those products are not the sponsor’s trial drug and are not covered by any trial data.

Will the Phase 3 trials answer the safety questions?

They will add much larger numbers and a longer follow-up to week 64, including a cardiovascular trial of 2,500 planned participants. Results are not expected before the registered completion dates, and registry dates can change.

How does petrelintide timing compare with eloralintide?

Lilly’s eloralintide has six Phase 3 trials with the earliest registered primary completion in January 2028, which is earlier than petrelintide’s earliest date of October 2028. Neither company has announced a filing.

Bottom line

Petrelintide is in Phase 3, not approved, and no marketing application has been announced. The registry puts the first trial completions in late 2028 and the last in 2030, and FDA review adds 6 to 10 months on top of the sponsors’ own preparation, so 2029 is a floor for any approval, not a forecast. What is established is the Phase 2 evidence in the comparison guide. What is unknown is whether Phase 3 confirms it, how long follow-up shows, and when, or whether, a filing happens.
